MOTS-c is a 16-amino-acid peptide with a feature that sets it apart from everything else in this catalog: it is not encoded in the nuclear genome, but within the mitochondrial genome itself, in a short open reading frame located in the 12S ribosomal RNA gene. Its name is the acronym of that location: Mitochondrial Open reading frame of the Twelve S rRNA type-c.
It was identified in 2015 by Changhan Lee, Pinchas Cohen and colleagues at the University of Southern California, and published in Cell Metabolism. The finding had conceptual relevance beyond the compound: until then it was assumed that mitochondrial DNA encoded only thirteen respiratory chain proteins. MOTS-c, together with humanin discovered earlier in the 16S RNA, established the category of mitochondria-derived peptides, which act as retrograde signals from the organelle to the nucleus.

The described pathway is indirect and unusual. MOTS-c does not bind to a membrane receptor: it interferes with the folate cycle and with the de novo purine biosynthesis that depends on it. That interference causes AICAR, a metabolic intermediate, to accumulate, and AICAR phosphorylates and activates AMPK, the kinase that works as a sensor of the cell's energy state.
The original study reported a more than twentyfold increase in endogenous AICAR levels and consequent AMPK activation in HEK293 cells and in mouse skeletal muscle, which appears as the main target organ. Under metabolic stress, the peptide also translocates to the nucleus, where it participates in regulating genes with antioxidant response elements.
A physiological detail relevant to interpreting the literature: MOTS-c circulates in plasma, its levels decline with age in both plasma and muscle, and exercise raises them markedly. A study published in Nature Communications in 2021 reported in humans an increase of about twelvefold in skeletal muscle and 1.6-fold in circulation after exercise.

That said, there is a distinction that is often lost when reading about this compound and it should be highlighted: it is one thing for endogenous MOTS-c to increase with exercise and be associated with a better metabolic profile, and another for exogenous MOTS-c administration to reproduce those effects in humans. The former is documented; the latter has not been established in controlled clinical trials, none of which have been published.
